Lewis' middle name is Hembree (his mother's maiden name) and is shown as such on Oregon Births and Christenings 1868-1929, WWII draft registration, and California Death Index 1940-1997. Oregon Marriage Index 1906-1924 and cemetery headstone show a middle name of Herman. Since Lewis himself filled out and signed the WWII draft registration card, "Hembree" is the name he commonly used as his middle name.
